Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted.Required for all travelers. Preferred to be completed prior to start, can be completed onsite. PAPR will not be completed on site. WI and MI require 3M 1860 Regular, 3M 1860 Small or 3M 1870+. Updated annually.Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler.Flu season is October 1, 2024 through March 31, 2025. Exemptions/Declinations are accepted, but candidate will be required to wear a mask at all times.1st PPD within 1 year, 2nd PPD within 30 days of start.
