Location: New York, NY 10004
Schedule: 9:00 AM - 5:00 PM (70% in-office; occasional travel for home visits to Manhattan, Brooklyn, Bronx, and Queens for wound care consults)
Job Summary:
We are seeking a Registered Nurse with Wound Care Certification to provide high-quality care for patients with complex wounds. This role involves in-office work and home visits for wound care consults, collaborating with an interdisciplinary team to ensure optimal patient outcomes.
Key Responsibilities:
Assess and review wound care patients for the Home Care Program.
Conduct wound, ostomy, and continence assessments, creating individualized care plans.
Provide patient and family education on wound care, prevention, and treatment.
Guide field nurses and perform field visits for skill evaluations.
Write progress notes and ensure accurate documentation.
Participate in quality improvement initiatives and compliance audits.
Ensure adherence to state and federal home health regulations.
Requirements:
Licensure/Certification: Current New York State RN License; Wound Care Nurse Certification required.
Education: Bachelor’s Degree in Nursing.
Experience: Minimum 2 years in home health, post-acute care, or medical/surgical nursing.
Skills: Proficiency in Epic H2O, Microsoft Office, and strong organizational and communication skills.
