Job Details
The Registered Staff Nurse (RN) provides holistic care to customers and families.Focus on independent, collaborative and delegated nursing functions as described in standards and competencies specific to the assigned service area.Scope of practice may include the care of neonates, infants, children, young adults, adults, and older adults throughout the life cycle.The RN executes the Standards of Nursing Practice including assessment, nursing diagnosis, outcome identification, planning, implementation, and evaluation.The RN develops and revises the customer's plan of care, coordinates care among disciplines and delegates activities to other customer care providers as appropriate.The RN participates in shared accountability and decision-making for nursing practices.Variation of duties may result based on the assigned customer care unit(s).This RN is to support a Hospice On Call Team, working from home unless attending an in-home visit.

2+ Years of Hospice In Home Care Experience - RequiredEpic Experience - PreferredWork historySkills Checklist, current to 1 year, pertinent to the specialtyTwo Supervisory reference forms, taken in the last two yearsIf a candidate is not eligible to work in the USA, it must be noted on their profileALL RTO requests must be included at the time of submittal, NMH may not honor RTO requests after submittal
Schedule Information
Rotating schedule every 2 weeks:Week 1: M, T on, W, Th off, F, Sa, Su onWeek 2: M, T off, W, Th on, F, Sa, Su offShifts are 4:30p - 8:30a with part of the shift in a primary role and part of the shift in the secondary role.
Unit Specific Information
Nonbillable Hours: 4 hoursDress Code - ScrubsClinicians MUST list prior work history with North Memorial Health, Maple Grove Hospital, and/or Blaze Health LLC on their resumeMN DHS Net Study & Fingerprinting is required for ALL patient-facing positionsNMH has a strict compliance deadline of 4 pm on the Wednesday before start - there will be no exceptions.
Additional Details
Mileage will be reimbursed at the federal rate.Each shift has two people on call. One is listed as the primary and the other is the secondary.When in the primary RN role, the clinician will be the first to be notified by the call service team and will be expected to respond either through a return call or an in-person visit.The primary RN receives regular shift pay. In the secondary RN role, the clinician will be on call as a back-up if needed and will receive On Call pay ($10/hr).A laptop will be provided, but the clinician must be able to use their own cell phone.
