Job Requirements
9 Panel completed within 30 days of start date, preferred within 14 days of start.All engaged contractors must have page 27 signed form on file in Trinity VMS.Standard i9 completed by the agency.Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement.Attach copy of specialty exam must be completed within a year.Proof of vaccine Sept 2024 booster, declination or exemption(with agency letterhead).Attached scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out.Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later.Flu dates: 10/1/2024-4/30/2025.Upload a current, non-expired TDAP vaccine.Provide documentation from an exam within the last year that candidate is fit to work.
Additional Details
American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS.
