HOW YOU'LL MAKE AN IMPACT
Your Role:
* Performs Minimum Data Set (MDS), case management, utilization review, quality assurance (QA), and discharge planning duties.
* Conducts federally mandated patient assessments based on Centers for Medicare and Medicaid Services (CMS) guidelines, including appropriate care planning, interdisciplinary communication, and Agency for Health Care Administration (AHCA) documentation preparation.
* Promotes wellness.
* Is responsible for collecting integral data and compiling/transmitting data to CMS.
* Determine patients' needs based upon illness, diagnosis, prognosis, and social support systems.
* Contacts appropriate agencies to coordinate patient discharges into the community or long-term care settings.
* Ensures facility compliance with applicable state and federal regulation.
* Collaborates with other members of the healthcare team.
Reports To:
* Administrator, Rehabilitation Center (JC: 013501) [with delegation (as needed) to RN/Director, Rehabilitation (JC: 030101)]

* None
WHAT YOU'LL NEED TO APPLY
Required Education:
* Associate degree in nursing.
Preferred Education:
* Bachelor's degree in nursing.
Required Experience:
* One (1) year of relevant work experience.
Preferred Experience:
* Two (2) or more years of relevant work experience
Required Certification/License/Registry:
* Florida RN license.
Preferred Certification/License/Registry
* Minimal Data Set (MDS) certification or ability to obtain certification within one (1) year of employment.
