The Behavioral Health Coach - RN or LPN provides leadership in delivering individualized nursing care using critical thinking and problem-solving skills. This role involves collaboration with healthcare teams to implement strategies that enhance patient care and promote healthy lifestyles. The coach utilizes the nursing process and motivational interviewing while overseeing disease registries and promoting clinical practice guidelines.
Serves as a leader by demonstrating independence, assertiveness, critical thinking, problem solving skills, and expertise in ambulatory quality measures. Collaborates with the healthcare team to implement improvement strategies that guide optimal care to patients. Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients. Collaborates with patients, families and the care team using education, goal setting and motivational interviewing to promote a healthy lifestyle. Must be organized, self-motivated, self-directed, and autonomous; willing to teach the healthcare team; and seek professional development in areas of expertise. Works with the clinical team, oversees the disease registries and promotes the use of Sanford approved Clinical Practice Guidelines and Protocol Orders.
