This facility provides short- and long-term nursing and rehabilitation services to the senior population in our community. The facility is fully committed to providing our residents and employees with a true family home-like setting where all employees look forward to going to work to provide the highest quality health care services possible.
Physical Therapist Job Responsibilities:
· Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
· Provide supervision as needed for staff Physical Therapy Assistants in delivery of the PT plans of care.
· Assist as needed with patient screenings for the need for Physical Therapy.
· Communicate progress and any barriers toward PT goals with interdisciplinary team members.
· Complete timely PT documentation in compliance with regulatory licensing and payor requirements.
· Assist with maintaining both the rehab gym equipment and treatment area in a safe manner using good infection prevention methods.
· Provide and document patient family and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.
Education Experience and Licensing Requirements:
Current Oklahoma licensure as a Physical Therapist required.
