This single appointment form-based, micro-focused, exam requires no treatment nor follow-up.

Schedule:

4-5 weeks at a time
Provider can choose the dates and states he wants to travel

License Requirement:

None (Any state license will work), New Grad can also apply

Credentialing Time Frame: 3-4 weeks

Mileage, Lodging, and malpractice will be provided.
