SUBMISSION REQUIREMENTS :
- Board Certified (ABOS, AOBOS) - REQUIRED
- Active VA license - REQUIRED (must be currently active, will not accept providers needing to obtain license)
- BLS/ACLS - REQUIRED
- COVID Vaccinated - Preferred; must have by time of credentialing
- Clean background is highly preferred
- MUST STATE AVAILABILITY AT TIME OF NAME CLEAR
