Starting Salary Range: $59,773 - $62,987 + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change
RADIAS Health is expanding! We are hiring a Mental Health Practitioner to join our Community Competency Restoration team. In this role you will work with members of the team to assist clients in the criminal justice system in obtaining and maintaining an optimum quality of life consistent with client needs and strengths. Emphasis is placed on working with clients in community settings and directly providing the services necessary to assist clients in achieving their goals. Coordinate and monitor assessments, treatment planning, and follow up for clients. Serve as an advocate and liaison for and on behalf of residents' rights and benefits.
Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm
This position works hybrid - some work is done in the community with clients and other work can be done from home.
***Position is contracted until 8/2025, with possible extension and contract renewal***
Duties and Responsibilities:
- Work with the larger treatment team to provide services for clients. Support the development, writing, implementation, evaluation and revision of overall treatment goals and plans in collaboration with the client and the treatment team.
- Develop an individualized crisis plan coordinated with community emergency support services.
- Meet with clients to explain services, assess client needs, establish and implement a care plan to address those needs and arrange for resources necessary for community transition.
- Coordinate services with other county or community agencies including but not limited to Mental Health Case Management, Adult Protection, and community hospitals.
- Assist in the development of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to assist clients in the community.
- Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing. Will assist and /or teach clients independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting. Will assist clients with making and keeping appointments and any other tasks that are required through the client’s recovery process.
- Attend commitment and other court hearings as needed. Complete required court reports if requested.
- File reports with Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other mandated/required reports when applicable.
- Actively and assertively engage and reach out to clients’ family members/significant others, including landlords, employers and other treatment team members after obtaining client permission.
- Provide on-call crisis intervention as determined by the Program Director; this may include after- hours phone calls.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner
Candidate will have a Bachelor's degree in Psychology or Social Work plus 2,000 hours work experience providing services to adults diagnosed with a serious mental illness

No college degree and 4,000 hours supervised work experience in the delivery of services to adults or children with mental illness.
