Job Overview
The Lead, Community Health Worker (CHW) position will assist in leading all efforts in helping high-need and complex patients address real-life obstacles that keep them from staying healthy, and to ultimately improve health outcomes. Community Health Workers are based at the primary care clinics and work together with an Interdisciplinary Team to support patients in addressing the social factors that impact a patient's health. The Lead CHW supports the clinic based CHWs with training, creating and updating processes/workflows, identification of community resources, and assisting with CHW coverage when needed. The Lead CHW is primarily remote and will travel to clinics, member's homes, hospitals, shelters, and/or community destinations in order to provide patients and CHWs with support, training in outreach, assessment and documentation. The Lead CHW will act as a technical assistant for the CHWs, assist in quality assurance, data analysis, and support with highly complex patients.
Minimum Requirements
High School Diploma or GED is required.
Bachelor's degree preferred.
Minimum of 3 years of experience as a CHW in a community health based setting
Experience writing case notes and care plans using an electronic health record required.
Experience working with diverse populations required.
Must have access to a safe vehicle when traveling to a home environment.
Certificate or courses in Community Health Work preferred.
Familiarity with EPIC, Essette, and local community resources preferred.
Bilingual (Spanish/English) or another secondary language required due to population being served.
Compensation
$24.00 - $28.15 hourly
