Job Summary & Responsibilities:
Role may require liaison with facility Directors to align our team with their initiatives.
Assessment and Diagnosis: Evaluating residents for psychiatric conditions such as depression, anxiety, dementia, or psychosis.
Medication Management: Prescribing and adjusting psychotropic medications, ensuring they align with other treatments and medications.
Therapeutic Interventions: Providing psychotherapy or coordinating with therapists for emotional and behavioral health needs.
Care Coordination: Collaborating with interdisciplinary teams, including primary care doctors, nurses, and social workers, to develop and implement comprehensive care plans.
Crisis Intervention: Addressing acute psychiatric symptoms, such as agitation or confusion, ensuring resident safety and well-being.
A strong understanding of policies, procedures, and regulatory State statutes of SNF's is a plus, along with experience related to compliance of coding, billing, prescribing, documentation and conduct.
Education, Licensure, and Experience:
Completion of a medical degree (M.D. or D.O.) from an accredited medical school.
Board certified
DEA license is required to prescribe controlled substances in Connecticut.
