Position Overview:
We are seeking a skilled and compassionate Physical Therapist PT to join our dynamic rehabilitation team. The PT will evaluate plan and provide therapy services to residents helping them regain mobility reduce pain and achieve their maximum functional potential. This role is essential to improving residents’ quality of life and promoting positive rehabilitation outcomes.
Key Responsibilities:
Evaluate residents' physical conditions and develop individualized treatment plans based on their needs and goals.
Administer and oversee therapy sessions including therapeutic exercises manual therapy techniques and mobility training.
Assess the effectiveness of treatment plans and adjust as necessary to ensure optimal progress.
Educate residents and their families on physical therapy techniques safety precautions and strategies for maintaining progress at home.
Collaborate with the interdisciplinary team including physicians nurses and other therapists to provide holistic and coordinated care.
Document assessments treatment plans progress notes and discharge summaries in compliance with facility and regulatory standards.
Maintain a safe clean and organized therapy environment adhering to infection control policies and procedures.
Participate in staff meetings in-service training and continuing education opportunities to stay current with best practices in physical therapy.
Qualifications:
Doctorate or Master’s degree in Physical Therapy from an accredited program.
Current and valid Physical Therapist license in the state of California.
CPR certification required.
Experience in a skilled nursing facility or rehabilitation setting preferred.
Excellent interpersonal communication and organizational skills.
Ability to work collaboratively in a multidisciplinary environment.
