Case Manager can expect to see 20-30 patients per week. Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.
EPIC/Computer charting. Sapphire IV pumps. T

Candidate MUST HAVE:
- 1 year Home Health RN experience
- Active BLS (AHA) and state RN license (IL)
- Wound care experience (wound vacs, pleural drains and ostomy care).
- Able to complete peripheral blood draws, central line care, and IV admin through multiple devices (elastomeric, IV pumps).
- Excellent assessment skills.
- Experience with computer documentation.
- Reliable transportation, active driver’s license, and insurance.
