POSITION SUMMARY
The Patient Navigator organizes, supports, and implements various Unio Health Partners (UHP) programs under the direction of the Team Lead and Program Director. This role helps to coordinate patient care and ensures that the patient understands and is knowledgeable about their specific condition(s). The Patient Navigator provides and documents non-face-to-face care coordination, mostly over the phone. Communication with physicians, staff, and patients is paramount in this position to ensure that patient care needs are met effectively and efficiently.
This is a remote position, and worker location must be within Unio Health Partners authorized states: AR, AZ, CA, CO, CT, FL, IA, NV, OR, TX, VA, WI
What you will be doing
- Enrolls and provides ongoing care coordination and patient navigation for patients in the Chronic Care Management (CCM) program over the phone and other non-face to face modalities.
- Functions as a liaison between patients, physicians, staff, and outside medical offices to improve communication and patient care.
- Coordinates patient specific needs that arise including, but not exclusive to, access to funding programs, psychosocial support, transportation or legal assistance, and clinical trial participation.
- Places and receives high volumes of patient calls daily, while remaining organized and diligent with any identified follow up tasks.
- Documents CCM care plans in the CCM Software and EMR, coordinates relevant follow up and communicates with providers and supervisors as needed. Care plans should be worked on daily and daily/weekly goals will be provided. These goals will be communicated by team leadership and may fluctuate based on program volume and patient needs.
- Coordinates cancer and medical care for patients, including patient education, program enrollment, appointment management, coordination of on-going diagnostics, medication adherence and adverse effect monitoring, and provider communication.
- Communicates with Team Lead, Program Director, and healthcare providers as appropriate to ensure timely communication about patient care and concerns.
- Provides a significant amount of patient/family education regarding cancer and medical treatments.
- Assists in reviewing medical records to identify patients that are appropriate for treatments.
- Assists in verifying insurance benefits and completing prior authorizations/appeals.
- Communicates in a caring and professional manner with patients, staff, and physicians.
- Actively advocates for patients to improve their healthcare experience and treatment outcomes.
- Initiates, monitors and coordinates care planning for assigned patients, including patients receiving immunotherapy infusions and those in our In-Office Dispensing Program.

What you will bring to the team
- Customer service experience in a call center setting strongly preferred
- Ability to be discrete and maintain patient confidentiality
- Strong communication skills: Exhibits excellent verbal and written communication via e-mail, telephone, and in-person in English
- Proficient typing and keyboarding skills
- Clinical experience especially with oncology, case management, patient navigation, nurse education and/or medical/urology/gastroenterology terminology strongly preferred
- Multitasking and creative problem-solving skills.
- Highly organized and self-motivated
- Ability to work independently on assigned projects
- Ability to prioritize tasks based on importance and deadlines
- Time Management: Speed is of the essence for patient navigators. Being able to manage how much time is spent on a project or patient is important
- Computer literate including Microsoft Word, Microsoft Excel, and electronic medical records
- Previous experience working in EMR systems
- Bi-lingual strongly preferred: Armenian, Russian, Vietnamese, Mandarin, Cantonese, Arabic

Compensation is depending on a number of factors including a candidate’s qualifications, skills, competencies, or experience that may fall outside of the range shown
Pay Range: $18 - $22 per hour
