Job Summary
External Description - Responsible for the 360 clinical, logistic and administrative oversight of Home Hospital operations. This role has daily oversight of Charge Nurse Team and is responsible for thru-put, capacity management and resource allocation to serve our HH patient population.
This is a unique role that partners with paramedic logistic team and dispatchers, to meet the demands across our geographic service areas.
This pivotal role will translate the objectives, policies, and procedures of the Hospital into effective action. The Clinical Operations Supervisor has the authority to make decisions regarding patient care within the framework of established policies. This role will have operational oversight over our full geographic region, encompassing 5 hospital systems.
About Home Hospital This innovative, multifaceted role will serve in the capacity as command operational supervisor. This role will focus on the following areas, allowing for full visibility into our growing and robust operations. Triage Role- Acts as the central contact person in coordinating high level patient placement and patient access for the HH system. Coordinates and collaborates with charge nurses, MIH leads, Apps, Navigators, Case Management, Medical Assistants and Physical Therapists to optimize capacity and streamline thruput. In addition, this role will be an escalation point of contact for our Remote Patient Monitoring Nurses. · Supports the facilitation of admissions and discharges as needed by the team. · Serves as key point person for team reporting and clinical operation troubleshooting. · Anticipates/proactively responds to capacity demands, as necessary. · Responds to operational safety events. · Assists with activated DEPC Operations Resource Role– Facilitates access across sites and point of entry. · Deploys clinical resources, coordinating staffing resources to respond to demand across the system. · Collaborates with Emergency Department, Medical/Surgical Units, Observation Units, PACU to facilitate access to Home Hospital. · Serves as Clinical point of contact for HH across all sites for support as needed. · Assists with staffing, filling open shifts to meet expected and surge demand as needed. · Oversight of Safety events, both systemic and local as needed. Patient Care Management- Serves as a clinical resource in the management of patient care. · Performs support to clinical team for escalated issues requiring clinical expertise and judgment. · Provides clinical consultation to staff and real time debrief, if needed – escalating to Managers, Executive Director of Nursing and Patient Care Services · Assists staff in managing daily activity. · Collaborates with other departments to facilitate/support patient care utilizing current policies and procedures. · Acts as a patient advocate, performing service recovery. · Acts as a direct care provider in clinical situations, aswarranted. · Initiates huddles with staff at beginning and end of shift to improve inter-departmental communication. · Mitigates operational concerns with 3rd party vendors, including but not limiting to Couriers, Remote Patient Monitoring, etc. NOTE: Position is located at Assembly Row in Somerville
Qualifications
Qualifications · Bachelor’s degree and current MA RN license required. · 5+ years nursing experience with progressive supervisory experience required. · Experience as a House Supervisor overseeing broadband of hospital operations or similar operating model required. · Proficiency with triage and managing multiple competing priorities. ·
Must have demonstrated clinical competence in area of nursing practice and demonstrated leadership ability. · Proficiency with Epic · Ability to Learn Paramedic Dispatch Software for awareness. · Excellent communication skills required, as well as spirit of innovation and agent of change.
