Welcomes patient and assist with move-in process. Deliver Welcome Packet, answer questions, and begin C.A.R.E.S. process. Coordinate communication with the Family, Nursing Home Staff, Social Workers and team regarding the admission process. Complete initial plan of care in EMR. Complete head to toe assessment and ensure that all appropriate treatments and orders are in place. Add any wounds to weekly wound log. Initiate appropriate preventative measures (i.e., skin, fall risk, elopement, etc.). Initiate all appropriate admission paperwork including initial plan of care, med list, admit papers, Physician orders with appropriate dates and signatures, code status papers and other required documentation. Verify orders with physician. Obtain and enter into EMR patient's current immunization status. Offer / administer / document immunizations as appropriate. Demonstrate familiarity with policies of the agency and rules and regulatory requirements. May be assigned other duties as appropriate.
Job Qualifications
- Graduate of accredited school of nursing
- Bachelor's and/or Master's degree preferred
- Current nursing license in good standing in the state you will be working
- Minimum two years of varied work experience as a professional nurse and preferably one of the two years in a post-acute setting
- Demonstrates skill in administrative performance including expertise with both written and oral communications and documentation requirements
- Demonstrates ability to assess and respond to the needs of patients and families in varied settings
- Demonstrates skill in nursing practice
- Ability to cope with family emotional stress and tolerance of individual lifestyles
- Ability to lift, transfer, and/or move items or patients with weights of 10 - 200+ lbs and who vary with ability to assist using proper lifting techniques and asking for assistance when necessary
- Ability to perform critical thinking and psychosocial assessment and judgments
- Manual dexterity and skillful use of hands when working with patients and equipment
