Veteran Directed Care Program Specialist

Reports to: Veteran Directed Care Program Coordinator

This position meets with Veterans who qualify for the home care program. They will complete a home visit with the Veteran complete required paperwork to enroll the Veteran. They will be required to do quarterly visit with the Veterans to make sure the program is working for all involved.

Requirements:

Follow up on referrals received from the VA within 5 days and complete Intake within 10 days.
Create monthly budget/annual spending plan according to VA approved case mix amount.
Conduct Person-Centered Planning and self-advocacy training to participants in the Veteran Directed Care Program.
Develop positive working relationships with veteran's, their families, caregivers, and VA contacts.
Provide information and resources about relevant services provided under the VDC program as well as counsel veterans on home care budget to ensure they are managing budget correctly.
Complete quarterly and annual home visits as required by the VA in a timely manner.
Complete thorough documentation of all encounters, Maintain all physical and electronic records in files, and databases.
Onboarding of Personal Care assistance in Paylocity platform, addressing payroll concerns and follow up on missing or incomplete onboarding items.
Ongoing communication with stakeholders, co-workers, and supervisor.

This position may work remote but is required to travel to the following counties: Franklin, Clinton, Essex, Hamilton, Warren, Washington County

Qualifications:

Bachelor's degree in human services or related field plus 2 years of experience – or – associate degree with at least 4 years of related experience
Able to pass a criminal background check
Excellent customer service and communication skills
Proficiency with Microsoft Windows Excel a must, Office, and with Zoom, Microsoft Teams and virtual meeting platforms
Organized and self-motivated able to work independently.
Must have valid driver's license and be able to provide own transportation within the following county region (Albany, Rensselaer, Schenectady, Saratoga, Columbia, Green, Montgomery, Fulton, Schoharie) will be reimbursed for mileage.
Personal experience working with the disabled individuals.
Understanding and acceptance of Independent Living Philosophy or willingness to learn about it.

Salary $45,000 - $48,000 per year
